In order to receive a certified copy of a birth or death certificate, you must be a qualified applicant (a parent, grandparent, sibling or yourself) and you must also present valid identification.
Requesting by Mail
To request a certified copy of a birth or death certificate by mail please complete the correct form (PDF) and attach a photo copy of your driver's license. The form must be filled out completely and signed. Requests can be sent along with a check or money order to:

- Birth Certificate: $23
- Death Certificate: $21 for the first and $4 for each additional requested at the same time
Notice
Effective September 1, 2015 all applications for certified copies of Birth and Death certificates which are mailed into the County Clerk's Office must provide notarized proof of identity per Health and Safety Code, Section 19131.
